PART of the PCT's strategy aims to improve access to health services for the 200,000 people it serves in Kennet and North Wiltshire.
It is planning to provide core services either at expanded GP surgeries, health centres or community hospitals.
These services include terminal care, intermediate care, nursing home and residential home care for older people, chiropody, community occupational therapists, physiotherapists and community services provided by the voluntary sector.
The trust plans to provide these services by developing emergency nurse practitioner roles for minor injuries, extending district nursing care to a 24-hour nursing service and increasing the capacity of its intermediate care services.
It also wants to develop a single assessment for patients and improve co-ordination between health care agencies.
It also aims to develop a transport strategy with Wiltshire County Council, voluntary link schemes and Wiltshire Ambulance Trust.
In particular, it plans to work with the county council, Kennet District Council and others to develop new partnerships in Pewsey and Ludgershall.
It will also work with GPs in Corsham to develop a health provision strategy.
